At the Living Center at Sunny Vista, we are proud to offer veterans care for skilled nursing and short-term rehabilitation through a new partnership with the Department of Veterans Affairs (VA). This collaboration allows us to provide essential healthcare services tailored specifically for those who have bravely served our country, delivered with the compassion and respect they deserve.

Our VA program at The Living Center is built around personalized care that addresses each veteran’s unique needs and experiences. We understand that every individual brings a distinct history and set of health challenges, which is why our skilled healthcare team is dedicated to creating tailored rehabilitation plans that promote recovery, dignity, and well-being.

What is Veterans Community Care?

  1. Who is eligible for services under the new VA contract?
    Veterans who are 70% service-connected or deemed unemployable (such as those on hospice) may qualify for services. We encourage veterans to follow up with their community social worker to review their individual eligibility and care options.
  2. What services are available for veterans through the VA partnership?
    Veterans with a service-connected designation may receive short-term rehabilitation, skilled nursing care, and respite services at our community. At this time, the VA does not cover assisted living or memory support services.
  3. What is the admissions process for veterans using the VA contract?
    We begin by offering the veteran and/or their family a tour of our community and a conversation to understand their care needs and determine if we’re the right fit. To move forward, the community social worker must be contacted to obtain approval and submit a referral to our Admissions team for review.
  4. How quickly can a veteran be admitted under the VA contract?
    For short-term rehabilitation, veterans may be admitted as soon as they receive VA approval and are medically cleared for discharge from the hospital. Admissions for skilled nursing may take a bit longer, depending on VA approval timelines and suite availability within our community.
  5. Who should families contact to learn more?
    Veterans and their families should start by speaking with their community social worker, who can help determine eligibility for VA-covered long-term care. If eligible, the VA will send a referral, and families are encouraged to contact our Admissions team at 719-291-9692 to schedule a tour and discuss the next steps in the move-in process.
